The circumstances of the accident should be noted in the central column in paragraph 16. For example, if the driver was overtaking, then two boxes should be noted: "changed lane" and "overtook". On the left are items for driver "A", on the right are items for driver "B". At the end of point 16, you must indicate how many boxes are marked by each driver.

In paragraph 17, you need to draw a road accident diagram. You don't have to be an artist for this. It should have a road with signs, markings, traffic lights and landmarks. Street name is required. Leaving the scene of an accident is an administrative offense, according to which the offender must be held liable. Since no amendments and changes were made to the Code of Administrative Offenses in 2015, then in 2016-2017 the violator will be punished in accordance with Part 2 of Art. 12.27 of the Code of Administrative Offenses.

These punishments can be of two types:
- deprivation of a driver's license for up to one and a half years
- arrest up to 15 days

How much damage can be covered

The back side of the form is filled in after the Notice is divided, i.e. each driver separately. The driver can indicate any additions, notes and notes on it. In case there is no more space, it will be necessary to write “with attachment” on the form and add what there was not enough space for on a simple blank sheet by attaching it to the Notice.

If the completed form is damaged, lost or one of the parties wants to make some changes to the information, a new form should be filled out.
