First documents of a newborn

1. Certificate of birth - the first document for a newborn, which is issued in a maternity hospital. The birth certificate confirms that the child was born to this particular woman. It contains basic information about the baby (his gender, date and time of birth, city, region and information about the hospital. This document is required for registration of a birth certificate and is valid for 1 month.

2. Exchange card (second and third sheets).The second sheet is "Information from the maternity hospital, the maternity ward of the hospital about the postpartum woman," which contains information about the course of childbirth and the postpartum period and the state of health of the woman. It should be taken to your gynecologist.

The third sheet - "Information of the maternity hospital, maternity ward of the hospital about the newborn" - contains information about the child's condition: gender, height, weight at birth and at discharge, assessment of the child's condition; the state of health of the newborn; information about feeding, about vaccination.

This part of the exchange card must be submitted to a medical institution where the baby will be monitored in the first years of life.

3. Third coupon of the generic certificate.A coupon for a birth certificate is not issued in cases where the management of pregnancy and childbirth was carried out outside the compulsory health insurance system. In this case, the children's polyclinic will issue itself the coupons of the birth certificate for the observation of the newborn.

Ticket number 3-1 is intended for payment to health care institutions for services for the first six months of dispensary observation of a newborn.

Ticket number 3-2 is intended for payment of services to health care institutions for the second six months of the child's dispensary observation.

The coupons of the birth certificate must be submitted to the clinic where the newborn will be observed.

Birth certificate

To obtain a birth certificate, you need to contact any registry office within one month after giving birth. There are no fees for registration of the certificate. One of the parents can apply to the registry office, while you need to have with you:

  • a birth certificate from the hospital. In the event that the birth did not take place in the maternity hospital, you will need a statement from the person who was present at the birth of the baby. The application is drawn up personally by this person at the registry office. If this person cannot appear at the registry office, his signature on the application for the birth of a child by this woman must be certified by the organization in which the specified person works or studies, the housing maintenance organization or body local government at the place of his residence;
  • documents proving the identity of the parents or one of them, if the family is incomplete;
  • marriage certificate, if any;

A birth certificate is a document where the baby's name is indicated for the first time, so this issue must be resolved before going to the registry office.

In addition to the birth certificate, in the registry office, parents are provided with a certificate that is necessary to obtain lump sum for the birth of a child. The certificate is valid for six months. It must be provided at the place of work of the father or mother or to the department of social protection of the population, if the parents do not officially work anywhere.

Compulsory health insurance policy (MHI)

This document is necessary from the very first days of life if medical monitoring of the baby's condition is carried out in a state polyclinic. A nurse and a pediatrician who come to care for a newborn in the first days after discharge will explain how to issue a compulsory medical insurance policy. Usually, for this, it is enough to contact a children's clinic or an insurance company, which one will be specified by the local pediatrician. To obtain a medical insurance policy, you need:

  • single parent's passport;
  • child's birth certificate.

Registration

The baby can be registered at the place of residence of the parents or one of them. If the parents are registered at the same address, then it is necessary to provide to the passport office:

  • passports;
  • birth certificate;
  • marriage registration certificate;
  • statement.

If the housing is not owned by the baby's parents, but the parents or one of them are registered, then the owner's permission to register the child is not required.

If the parents are not registered together, then the child can be registered at the place of residence of the father or mother. In this case, the second parent will need a statement that he does not object, and a certificate confirming that the child is not registered at the place of residence of the second parent. In addition, you need an extract from the personal account and home book of the second parent, whose baby is not registered.

If the family wishes to register the baby within a month after the birth of the mother, her application will be sufficient. If you miss this deadline, then the father will have to bother to obtain the above certificate and an extract from the house book.

If you do not register your baby within 1-3 months, a fine may be charged for living without registration.

Pension insurance certificate (SNILS)

SNILS should be issued in the department The Pension Fund RF. It is required when making any payments for a newborn. Therefore, the sooner this document is completed, the better.

For its registration you need:

  • identity document (parent's passport);
  • statement.

It should be noted that the introduction of information technology greatly facilitates the preparation of documents: at first, it is enough to declare the need to obtain documents on the relevant Internet portals, and then receive the document in person.

In addition to drawing up the necessary documents, after childbirth, you need to worry about calculating all payments and benefits due.

Maternity allowance for working women

This allowance, as a rule, is paid even during pregnancy immediately after the woman provides a certificate of incapacity for work, which is issued at 30 weeks for a singleton pregnancy and at 28 for a multiple pregnancy. However, in the case of complicated childbirth, the maternity hospital will issue the woman an additional certificate of incapacity for work for 16 calendar days, which will also be paid.

Childbirth is considered complicated if, for example, it ends with a cesarean section, if a manual examination of the uterus is performed after childbirth, in multiple pregnancies, etc. In the case when the diagnosis of multiple pregnancy is established in childbirth, a certificate of incapacity for work for pregnancy and childbirth is issued for an additional 54 calendar days (not 16 days) by the medical organization where the birth took place.

It happens that there are several reasons to recognize childbirth as complicated. For example, with a predicted birth of twins, a woman is initially issued a certificate of incapacity for work not for 140 days, but for 194 calendar days. That is, even before she goes on maternity leave, she is paid a sick leave of a longer duration. If she also underwent a caesarean section, a certificate of incapacity for work for an additional 16 days is not issued and is not paid.

When extending the certificate of incapacity for work for 16 days, it is necessary to submit it to the accounting department along with an application for the extension of maternity leave and the appointment of maternity benefits.

Other payments

In addition to federal payments for the birth of a child, regional benefits are provided. They are paid from the budget of the subject Russian Federation, and their size differs in different regions of our country. It should be noted that in some areas, payments for the birth of a child are not provided.

After the birth of the second child, regardless of the region, the parent is entitled to maternity capital. It is intended for intended use and is issued as a certificate. It can only be used to buy or build housing, pay for children's education, and increase the mother's pension. You can issue a certificate at the branch of the Pension Fund of the Russian Federation by providing following documents:

It's time for kindergarten

When all the documents are completed, it is better to take care of putting the baby on the waiting list for a preschool (kindergarten). This event does not have strictly specified, normalized deadlines, but it should be borne in mind that the queue can be impressive and last several years - just as long as it will take from the birth of the baby to the time of the opportunity and the need for him to attend kindergarten.

In most regions, a special service of the Department of Education deals with the distribution of places in preschool institutions. You can submit documents both in person and in absentia using the appropriate website. The following documents will be required:

  • child's birth certificate;
  • information about the child's registration address at the place of residence or stay;

The above list of documents is enough for a child up to at least 14 years old. And many documents from the list are drawn up once in a lifetime, so having made all the documents necessary for a Russian citizen for a newborn, you will forever save yourself and your offspring from further hassle of obtaining papers.

The card indicates: weight, height of the child, vaccinations After discharge from the hospital, the child must visit the district doctor and the visiting nurse.

  • Help extract.

This is the main document confirming that the child was born by you. It says when and where the child was born, what gender the newborn was, who took delivery. This certificate is valid for 1 month.

You need to take it with you to the registry office for registration of the child's birth certificate.

Note: If a newborn was born outside medical organization and without the participation of doctors - take a statement from the person who was present at the time of the birth of the child.

Birth certificate

In fact, a newborn can be registered at any registry office in the city, but most often the child takes place in the registry office at the place of residence of one of the parents. Lump-sum child allowance

The size of a lump sum for the birth of a child is determined Federal law "On state benefits to citizens with children" from 19.05. 1995 No. 81-FZ (as amended on 05.12.2006)

Registration

Usually the child is registered at the place of residence of one of the parents. To do this, you need to provide the following documents to the passport office:

For newborns born after February 2007, the collection of documents for registration of the child's citizenship was simplified as much as possible. To obtain Russian citizenship, it is necessary to hand over the parents' passports and the birth certificate itself to the district department of the FMS. The stamp is affixed directly on the day of application on the back of the birth certificate in the upper left part.

Keep in mind that to obtain all the documents, you may need not only the originals of the documents, but also their copies, so at the very beginning of this documentary epic it is better to make several copies of each document.

In the postpartum rush, one must not forget about one's own health, therefore, a visit to an obstetrician-gynecologist must be included in the list of things to do. This visit cannot be considered formal in any way - you should visit the doctor not only in order to hand over an exchange card. After a spontaneous birth, you can go to the antenatal clinic about a month after giving birth. After a caesarean section, this must be done in the first week after discharge from the hospital. If you have any complaints, you should contact your doctor immediately. The doctor will assess how the recovery process is going, and if there are stitches, how they are healing. You can consult with your doctor about the possibility of resuming sexual activity, discuss contraception options, and maybe discuss the timing of planning your next pregnancy.
